Notes:
-It’s essential that our prayers are offered in the name of Jesus Christ. Latter-day Saints almost always conclude their prayers in Christ’s name, but technically you could express that sentiment at any point in the prayer. It may not exactly be “orthodox,” but I don’t think God is too concerned about order of operations here. He just wants you to be sincere in your prayers.
-You may have noticed in my example prayer that I used words like “thee” and “thou” instead of “you” and “your.” Most Latter-day Saints use those more formal terms as a sign of respect, but that is also largely cultural. For example, in Spanish, most Latter-day Saints use the personal “tu” form instead of the formal “Usted” form, as it makes their prayers more personal and actually removes barriers of formality, which I think is really cool. You do whatever you feel most comfortable with, or try out multiple ways of addressing your Father in Heaven and decide what feels best.
