Stake Presidents pray over who they think are potential candidates. Then they give it to the 1st Presidency who then prays and comes up with a name (if its even on the list, if not they know by inspiration). Then the 1st Presidency calls and asks if they will serve as a Mission President. I know my mission president was called by President Hinckley while he was driving on the freeway. :)
