How do men's garment sizes come? In other words, do they run large or small? If I have a size 40/41 chest, should I get a medium or large top? If my waist is a 35, should I get a size 34 or 36 bottom?

Large top and if you like the bottoms tight 32-34. If you like them looser like boxers then 36/38. Buy one pair and see if they fit like you want them too. Ask for help with fabrics. Also remember they come regular length and tall for men.

My husband works construction. He prefers the nylon mesh for all seasons.

yeah the best answer is to make a rough guess and try them out and then adjust to what your comfortable in.

Went yesterday to pick up temple clothes. Mentioned I didn't like the Carinessa II bottom in the hot months and suddenly found myself covered in tape measure.

I think a good employee at the distribution center will do the same thing for you. If you're ordering online, I think they have a measurement guide there.

As noted online has a sizing chart, and the garment packaging itself gives you sizing guides (So X size is for Y-Z measurments), and of course as also noted, the employees can look at the packaging if it isn't a distribution center where you can browse the shelves yourself.
