I know you can get your patriarchal blessing on lds.org, but is there a place you can get the dates of your ordinances? Many of my records have been lost and I'd like to make a note of the dates again.

If I were you I would call the church office in Salt Lake and speak with them. You might get the number from your Ward Clerk by speaking with him.

I would doubt your records were lost.  I think you just haven't found where they are.  And had them transferred, and all that.  Your ward clerk may be able to help, or you may have to go to or call Salt Lake City.

Does your Ward have your membership records?  The ordinance dates should be on your membership records and you can ask the clerk in your ward to help you with the dates.
