The LDS prayer format addresses God the Father and Jesus Christ. Why doesn't it address the Holy Ghost or Holy Spirit?

Yes; but note that there is a separate thing called Light of Christ.

Well, we pray to God alone and not to Jesus (usually). We invoke the name of Jesus because He is our advocate; it is He who intercedes for us.

The Holy Ghost acts as a messenger of God. He is an integral part of the prayer process; but His role is on the receiving--not the asking--end of the prayer.

Prayers are addressed to God the Father in the name of Jesus Christ. (John 16:23, Luke 11:2)

Ordinances are done in the name of Jesus Christ (James 5:14)

Baptism is performed "in the name of the Father, and of the Son, and of the Holy Ghost"

The "spirit" may refer to the Holy Ghost, or it may refer to the Spirit of Jesus Christ. The sacrament prayer says "That His Spirit may be with them." There was a thread somewhere that discussed whether it was the Holy Ghost or the Spirit of Jesus Christ. I don't recall what the consensus was.

Holy spirit and Holy ghost are synonymous 99% of the time.

well our prayers address God directly- He's the one in charge, as well as being commanded to... and the reason we do it in the name of Christ is because he's the one that brings us to God, and that we have taken upon us the name of Christ.

We are not commanded to to pray to or in the name of the Holy Ghost, nor is it the Holy ghost that atoned for us.
