So when does something stop being discipline and start being punishment?

I never once called it punishment....it should always be discipline

I personally know a woman who was excommunicated for fornication, specifically for sex with her boyfriend (later husband). I think the deciding factor is whether there is intent to repent or just to keep on living the lifestyle.

I think that is probably a pretty important factor of deciding whether or not someone gets excommunicated in situations that don't call for an automatic excommunication.

It would seem so.  (And properly so, in my opinion.)  After all, the main purpose of any disciplinary action in the Church should be as a call to true repentance; one who is already truly repentant doesn't call for as much action, if any beyond guidance, while one who is not repentant at all will require much more.
