I wouldn't be all that concerned about it, seeing as the specific case you site happened in 1989 and lasted for about 11 months.  At the time, the Church counted roughly 9,000 members in Ghana.  In 2014, it counted 60,000.

I don't see this happening in very many places in the world (excepting theocracies)

It seems to me that we’re pretty close to such a situation in Russia.  Getting a temple dedicated there will be quite the accomplishment—at least, if it is given the fanfare and publicity the Church ordinarily gives to new temples.

It has happened several times.

Here are some of the countries we used to preach at, but which that have banned preaching by the LDS Church:

Israel; Iran; Lebanon; Laos; Syria; and Vietnam
