The ward I belong to, is run by what I like to call "the ruling dynasty" (tongue in cheek of course). 
To give you an example of what I mean, The bishopric, Elders quorum, Young Women's and primary presidencies (plus their councillors) are all people from the same family more or less - only the relief society has been spared - and we don't have a Sunday school president so that's safe too I guess! 
 

There are a few reasons for the current situation and I can assure you it's not nepotism. I'm a firm believer that the Lord chooses his officers. (Maybe I'm just biased because it's also my family). Over the years it's lead to a lot of ire from other congregants.

Is this a fairly common scene across the Church, or are these just exceptional circumstances? (I don't travel much)

I've seen husband and wife called as bishop and RS president, but never as many as this from one family.  But then, I'm in Utah, we aren't exactly short of worthy members willing to fill callings (though it does seem like we're getting shorter).

I once gave a talk in Sacrament meeting on the same day I taught RS.  It was back when we did three hours and I joked that if only they'd asked me to teach Sunday School that week any visitors would have thought they were at the Church of Zil. :D

Haha. I'm sure it was a good Sunday, I've done lessons and talks before, but in reality I find bearing testimony on a fast Sunday exhausting, can't imagine a talk and lesson combo! How did you prepare for it?
 

my wife always says I missed "the good old days" as our ward has shrunk considerably over time - which partially explains it. It's also the fact this particular family has been around since the ward was built in the 60's and is a bit geographically isolated.

I've known similar situations. You get an inner clique of people who run nearly everything. You get a similar thing  in student Christian societies too, when the officers choose their own successors for the next year.

Yeah it does certainly look like this at times. That said our congregation gets just shy of a hundred on a good week, and almost all of the worthy and regular attendees are put to work one way or another. 
Just most of the big "titles" if you will all belong to the same family.

FWIW, in at least one version of the handbook, it says that this should be avoided if possible, but of course, the Lord does what he does and works with whoever he has available to work with...

:) I just did?  I love teaching the gospel and giving talks, so it was no problem for me.  I'm sure they gave me plenty of time to prepare, too.

At one point in time, I was the gospel doctrine teacher and an instructor in the high priests group. One Sunday I was asked to give a talk, so I gave a talk the first hour, taught gospel doctrine class the second hour, and gave the high priest instruction the third hour. People joked that it was All Vort Day.
